Order Commitment — Submission of this order form indicates serious intent to establish an on-site clinic. The needs assessment fee is non-refundable to cover initial consultation and evaluation costs.
Cancellation Policy — If the client cancels before site assessment, no further costs apply. If the client cancels after site assessment but before clinic setup, they forfeit the needs assessment fee. If the client cancels after clinic setup has begun, they must cover all incurred costs, including materials and logistics.
Service Contract Requirement — After the assessment, a formal contract will be provided for signature before service operationalization.
Customization & Setup Timeframe — Site setup timelines depend on the selected service package and will be communicated during assessment.
Payment Terms — Payments for setup and services must be made per the agreed schedule in the final contract.
