Order Commitment: Submission of this form indicates serious intent to establish an on-site clinic. The needs assessment fee is non-refundable to cover initial consultation and evaluation costs.
Cancellation Policy: If cancelled before site assessment, no further costs apply. If cancelled after assessment but before setup, the needs assessment fee is forfeited. If cancelled after setup has begun, all incurred costs must be covered.
Service Contract Requirement: After the assessment, a formal contract will be provided for signature before service operationalization.
Customization & Setup Timeframe: Site setup timelines depend on the selected service package and will be communicated during assessment.
Payment Terms: Payments for setup and services must be made per the agreed schedule in the final contract.
